One letter from the Department Chair, that attests to the quality of mentoring and coaching, and the extraordinary effort contributed to students by the nominee; and the impact of and mentoring on the student(s). Letters are to be addressed to Jenny Patrick, 233 Mabel Lee, (0234) and will not be returned to the nominee.
2. A letter from two students.
3. Earfa report of faculty activities must include up to a maximum 2 years, including the current year.
4. Nominee prepared materials (no more than 2 pages) including related to their creative efforts relative to coaching and mentoring.

Faculty Student Mentoring Award:
